Introductory Care Package

Our Introductory Care Package is designed to get a complete picture of your health and medications, so we can create a personalized care plan just for you. All patients begin here with a chance to continue on to our membership options after completion. This package is 2 months of care (starting from the date of the first introductory visit). Each visit builds on the last- from understanding your meds to creating and refining a plan that actually improves your experience. We turn confusion into clarity and give you a plan you can follow with confidence.

• Two visits within the same week:

  • Visit 1 - Full Medication Discovery & Priorities:

    We take time to understand your health history, medications you take (including OTC and supplements), and how they fit into your daily life.

    We talk about what matters most to you - your goals, concerns and any symptoms you’re noticing.

    • You leave with: A clear picture of you medication profile and what we’ll focus on next.

  • Visit 2- Personalized Medication Review & Action Plan

    We review every medication for safety, effectiveness, interactions, side-effects and overall fit with your health goals.

    We create an easy-to-understand, prioritized plan just for you.

    • You leave with: A written medication plan, clear next steps, and support for talking with your providers/pharmacy.

• Visit 3- Follow-Up & Reassessment (a third in-person visit over the 2 month period).

  • See how the plan is working.

  • Tweak recommendations if needed.

  • Answer questions from you or your caregiver.

  • You leave with: confidence in what’s working and clarity on next steps.

• Unlimited Pharmacist Access Via Phone, Text & E-mail (Monday-Friday, 8am-5pm & Sat-Sun 9am-3pm). This is available for the two month period (starting from the first introductory meeting).

  This package gives you peace of mind, personalized guidance, and a solid foundation for ongoing care if you choose to continue with a monthly membership.
