  • Comprehensive MTM review (a personalized review of all your medications to make sure they’re safe, effective, and easy to manage).

  • One Monthly Home Visit

  • Unlimited Pharmacist Access Via Phone, Text & E-mail (Monday-Friday, 8am-5pm & Sat-Sun 9am-3pm).

Guardian Membership

Designed for seniors and caregivers who want continuous, personalized pharmacy support that have an extensive medication list and need frequent medication reconciliation due to doctor visits/hospital admissions. This option works well for patients who would like us to continuously fill their pill boxes for them. Billed monthly.

$350/month

$3,850 annually (save $350 with annual plan)

  • Two Monthly Home Visits

  • Priority Scheduling

A La Carte Services

In combination with a membership we also offer:

  • Medication Pick Up & Delivery - $30 per trip within 10 miles of your home.

  • Additional In-Person Home Visit (1 hr) - $150 per visit
